The regulator determined that Welsh Water, known as Dŵr Cymru Welsh Water, failed to properly operate, maintain, and upgrade its wastewater network to ensure it could cope with the levels of sewage and wastewater. Additionally, the company did not have adequate processes in place or sufficient oversight by senior management.
